LETTER OF AUTHORIZATION
 
The Letter of Authorization is required by Lenders to confirm that they are authorized by their Borrower to
release information and communicate with the facilitating party.  This form must be completely filled out and
signed by the Borrower in order for any additional party to communicate with the Lender on the Borrower's
